A pilot project by the Hamilton Central health link is being lauded as a success when it comes to addressing the heaviest healthcare users in the area.
The Hamilton LHIN is one of 19 in the province working towards having better understanding of why 5 per cent of patients use two thirds of the resources.
Healthcare learning circles implemented by the group cut emergency room visits and hospital admissions for the 322 patients who use up the majority of the city’s health care expenditures by 10 per cent.
The study found that of the 322, two thirds moved off the list within the year.
The pilot project found that the majority of patients were in their last year of life, highlighting the need for better care planning.
